QUESTIONNAIRE

Please submit an application if you are interested in a dog/puppy that is available. An application received when we are not planning a breeding will be saved for future consideration if approved.

Please fill out this questionnaire and answer each question as thoroughly as you can. You don't have to send us an essay, but remember: the more information we have, the more likely we are to approve your application. Once it's all filled out & submitted, we'll email you back from mythosk9s@gmail.com.

There are no right or wrong answers. Just be honest. 

Subscribe to our mailing list to keep up to date on planned breedings and any older dogs we may place. Don't check your email much? Add your phone number and you'll also receive a short text message stating the pairing.
